• CPT 99453,
• CPT 99454,

• CPT 99091

The Service Codes for RPM are CPT codes 99453 and 99454, respectively.

CPT Code 99453

CPT Code 99453 was created to help patients who aren’t tech-savvy, can’t navigate their phones, or have trouble utilizing medical devices. This service code helps individuals receive training on using these devices and compensates medical health providers for providing these services to patients.

Because the Centers for Medicare and Medicaid Services (CMS) does not reimburse for the pieces of equipment, this includes the time spent on patient education or onboarding. This code covers the RPM enrolment service code, which is reimbursable.

CPT Code 99453 can only be billed once when it is first set up. The same healthcare provider or National Provider Identifier (NPI) number cannot be used twice.

CPT Code 99454

After a patient has been enrolled in RPM and monthly services have begun, CPT Code 99454 is used. CPT Code 99454, which is used to alert transmissions and report the administration of healthcare devices for health-related data, is reimbursable for these services. The programming and delivery of health-related devices are covered by CPT Code 99454.

CPT Code 99457

CPT Code 99457 reimburses for up to 20 minutes per calendar month spent remotely monitoring physiologic measures such as weight, blood pressure, respiratory flow rate, pulse oximeter, etc. It also includes management services and time spent in interactive communication sessions with patients or caregivers by physicians, clinical staff, and other certified healthcare professionals during the month. The billing cycle for this code is once per calendar month.

CPT Code 99458

CPT Code 99458 is a monthly add-on code for CPT Code 99457 that reimburses health care professionals for each additional twenty minutes of interactive monitoring sessions. Each calendar month, this code can be billed up to two times.

CPT Code 99091

Although CMS still considers CPT Code 99091 valid, the codes listed above better depict the numerous medical circumstances requiring billing. It is limited to data acquired over the course of 30 days and cannot be billed in the same month as CPT Codes 99457 and 99458.

Significance of 2022 CPT Code Changes

The Centers for Medicare and Medicaid Services released the PFS for 2022 on remote therapeutic monitoring (RTM) codes. 5 new codes have been included in the RTM. Before discussing the 2022 coding changes and new implementations, let’s understand what Remote Therapeutic Monitoring and its codes mean. 

The Purpose of Remote Therapeutic Monitoring

Remote therapeutic monitoring in general has management codes that are specifically designed to mirror the remote patient monitoring CPT codes. Remote therapeutic monitoring’s (RTM) intended plan of action is the management of patients with the help of medically designated devices by collecting non-physiological evidence and data. Despite the coverage planned by CMS fr covering RTM as one of the proposed rules, the importance of the RTM concept is to note the specific requirements that are still undefined. 

The CMS has discussed two differences between RPM and RTM that are major. In RPM, providers cannot bill patients for RPM services but maybe provide a CPT bill for RTM. the proposed rule for RTM shows how Medicare has pointed out that the AMA designated RTM codes as non-evaluation and management (E/M) codes, which disallows Medicare to lace them as general supervision care delivery management codes. The second way RPM is different from RTM is the nature of the data collected. RTM can be worked for non-physiological healthcare devices, for example, support for medical adherence. Any information on a medical device that can be collected which are non-physiological and be billed under remote therapeutic monitoring. 

New CPT Codes for 2022: Remote Therapeutic Monitoring

The care delivery starts with RTM services being activated and comes to a conclusion when the goals of specific treatment are met. 
• 98976: RTM devices, especially needed for respiratory systems, require the CPT code to be supplied with scheduled daily programmed alerts and recordings transmissions are needed for monitoring respiratory systems every 30 days by proper providers. 
• 98977: Remote therapeutic monitoring devices for this CPT code are required to be transmitted with the supply of scheduled recordings and programmable alerts for transmissions for monitoring the musculoskeletal systems every 30 days. The codes 98977 and 98976 are billable once per 30 days of monitoring. 

98980 and 98981: RTM treatments, physicians/providers who are exceptionally qualified in healthcare delivery on time in a month are required to have at least one interactive communication with the caregivers of the patients and the patients during that month for at least 20 minutes of the said month via phone calls or video communications.

• CPT code 99091 is for the collection and interpretation of physiologic data digitally stored and/or transmitted by the patient and/or caregiver to the physician or other qualified healthcare professional. It can also be billed monthly but has more requirements than CPT codes 99457 and 99458.

Calculate how much Medicare pays per CPT code assigned to remote patient monitoring

It’s important to understand how much you can expect to get paid per patient for your remote monitoring services.

Here’s how remote patient monitoring reimbursement breaks down using average Medicare reimbursement:

• Using CPT code 99453, Medicare pays a one-time payment of $18.48 for initial patient enrollment into the program. This includes the setup of the remote patient monitoring device, the delivery of the device, patient education, and receiving initial remote patient monitoring services.
• Using CPT code 99454, Medicare provides a base monthly payment of $55.77 for continuously monitoring patient data transmitted from the device (like blood pressure or blood sugar levels) as well as ongoing management of the device.
• Using CPT code 99457, Medicare provides a monthly payment of $50.18 for 20 minutes of communication between the patient and caregiver or practitioner specifically pertaining to the data transmitted by the device.
• Using CPT code 99458, Medicare provides an additional $40.84 if the time spent exceeds 20 minutes but is less than 40 minutes. If you spend more than 40 minutes per month, Medicare will provide an additional $40.84 dollars. There is no additional reimbursement after 40 minutes.
• Using CPT code 99091, Medicare provides a monthly payment of $56.41 for collecting and interpreting the transmitted and stored physiologic data provided by the remote patient monitoring device. The amount of time spent interpreting and analyzing must be at least 30 minutes to qualify for reimbursement.

A single remote patient monitoring patient will earn your healthcare organization $2,928.48 per year, assuming you get reimbursed the maximum reimbursement per month.

If your practice enrolls 50 patients into a remote monitoring program, and, assuming you get reimbursed for the maximum amount you can each month for each payment, you’d earn $147,348 in annual Medicare remote patient monitoring reimbursement. Even after you factor in the cost of remote patient monitoring software, you’d still be yielding a significant and consistent amount of revenue.

 Key considerations: Realistically, your care organization will likely only meet the initial 20 minutes for reimbursement, and you also might not consistently meet CPT code 99091 each month, so it’s important to factor in how much time you can feasibly devote to each patient enrolled in the program when calculating potential revenue.

Understand which CMS guidelines pertain to remote patient monitoring reimbursement

You must understand the CMS guidelines for remote patient monitoring to reduce the likelihood that your claims get denied.

In order to even qualify for reimbursement, CMS expects your practice to follow these remote patient monitoring requirements:

• Charge Medicare Part B patients a 20% copayment.
• Receive verbal or written consent from a patient to receive remote patient monitoring services.

Remote monitoring of patients is a way for healthcare providers to access information about patients who are not in the provider’s presence. Healthcare providers gather data from their patients without requiring the patient to visit a doctor’s office and submit it to on-site testing.

Remote patient monitoring (RPM) is an established part of medical practice and billing. Billings for RPM have generally been limited to the collection and monitoring of specific data using medical devices, followed by virtual consultations with patients who are being monitored.

For example, patients who measure their blood pressure, pulse rate, or blood glucose at home use devices that transmit that information to their doctor. If the data alerts the doctor to an emerging or urgent healthcare problem, the doctor can contact the patient with advice or to schedule an office visit.

Remote monitoring of patients offers the promise of better healthcare at a reduced cost. Because the patient does not need to meet the doctor in person, RPM results in fewer office visits. Virtual care management meetings allow the patient to receive medical advice from the comfort of his or her own home. Staffing needs are reduced because a provider can manage health care without the need for staff to meet with the patient, bring the patient to an examination room, and juggle patients because of missed or late appointments.

What Are the CPT Codes for RPM?

For some time, Medicare and private insurance companies have offered reimbursement to healthcare providers who implement a system for remote health monitoring of their patients. To assure that physicians are reimbursed for the time they spend reviewing data that was gathered remotely, the government agency that handles Medicare and Medicaid reimbursement — the Centers for Medicare & Medicaid Services (CMS) — established CPT codes for remote patient monitoring.

The American Medical Association, together with CMS, creates and maintains 5-digit codes that describe each service or procedure for which a healthcare provider might bill. Insurance companies and government agencies rely on those codes to determine the rate of reimbursement the provider will receive for each billed procedure or service.

The CPT codes for RPM typically cover the monitoring of physiological measurements, including blood pressure, pulse oximetry, and respiratory flow rate. One billing code applies to a patient’s initial entry into an RPM program. The code covers the time spent demonstrating the data collection device(s) to the patient and explaining how often the patient should take a measurement.

A second CPT code covers 30 days of monitoring data, provided the patient takes measurements at least 16 times during the 30-day period. The code also covers the physician’s amortized cost of the data collection equipment that the physician supplies to the patient.

A third CPT code covers 20 minutes of remote contact with a patient by the provider, including doctors and care managers, to discuss healthcare management based on the collected data. That code is billable monthly. A fourth CPT code covers additional time spent in virtual interactive patient care during the month in 20-minute increments.

What is Remote Therapeutic Monitoring?

The success of remote monitoring persuaded CMS to expand reimbursement to monitoring of data that was not covered by its initial approval of CPT codes for RPM. The CMS has authorized a new set of CPT codes that govern expanded services that healthcare providers offer remotely. 

Remote therapeutic monitoring (RTM) codes cover services that are similar to RPM but include a broader range of health conditions. At the moment, RTM includes monitoring of respiratory and musculoskeletal conditions. 

The CMS rules governing reimbursement of RTM differ from RPM rules with regard to the kind of data that the provider monitors and the ways in which data is collected. The rules also differ with regard to the kinds of providers who are eligible for reimbursement.

Like RPM, data collected by medical devices is a part of RTM. For example, a physician may provide an inhaler to a patient with a pulmonary condition (such as COPD) that monitors when and how often during the day the patient uses the inhaler, how many puffs/doses the patient uses each time, and the pollen count and environmental factors at the time the patient uses the inhaler. The physician would use that information to determine the effectiveness of treatment, the environmental conditions that appear to trigger respiratory distress, and the possible deterioration of the patient’s condition. 

However, RTM also includes certain self-reported data. While coverage for RPM depends on the use of medical devices that record and upload the patient’s physiological data, some RTM data can be self-reported, entered manually into a device, and uploaded by the patient.

The new rules allow reporting of data that can’t be recorded by a device, such as pain levels and reactions to medication. At this point, the list of data that can be collected appears to be illustrative rather than exhaustive. It is not entirely clear how much therapeutic data (i.e., data that is not measured by a medical device) can be included in reimbursable RTM data monitoring.

The other key difference between RTM and RPM is that RTM is classified as general medicine, while RPM is classified as Evaluation and Management services. Classifying RTM as general medicine makes it possible for additional Qualified Health Care Professionals (QHCPs) to apply for reimbursement for remote monitoring services. The CMS anticipates that most billings using RTM codes will be for services provided by nurses and physical therapists.

What Are CPT Codes for RTM?

The CMS has authorized five new CPT codes to describe and seek reimbursement for RTM procedures. Those codes will go into effect on January 1, 2022.

To some extent, the codes parallel RPM codes. The first code addresses the initial set-up of RPM, including instructions on when and how to record data and how to upload the data to the provider’s system. 

The next two codes address monitoring during a 30-day period. Monitoring includes a review of recorded data as well as the patient’s adherence and response to therapy. One code applies to the monitoring of the respiratory system and the other applies to the monitoring of the musculoskeletal system.

The last two codes apply to interactive communications with the patient by a physician or other QHCP. The first code describes a 20-minute session while the second code applies to each additional 20-minute increment.
